Observation:

• A 9-year-old male with SOD, hypopituitarism, hypothyroidism, and developmental delay was scheduled for dental extractions.
• Pre-anesthetic preparation included sevoflurane induction, intravenous hydrocortisone and dexamethasone, and a 5% glucose infusion.
• The patient maintained spontaneous breathing and hemodynamic stability throughout the procedure under sevoflurane/air anesthesia with local lidocaine infiltration.

Findings:

• Continuous monitoring confirmed stable glucose levels every 30 minutes during the procedure.
• The patient received paracetamol for analgesia and was discharged within 24 hours post-procedure without any adverse events.
• The combined anesthesia technique successfully managed pain and prevented adrenal suppression.

Implications:

• This case demonstrates the feasibility and safety of a combined inhalational and local anesthesia strategy for dental procedures in pediatric SOD patients.
• Careful perioperative management, including appropriate steroid and glucose supplementation, is crucial for mitigating risks.
