Summary

Real-time strain elastography (RTE) did not significantly decrease nondiagnostic (ND) thyroid nodule fine-needle aspiration (FNA) rates in this study. Further prospective trials are needed to confirm if RTE-targeted FNA improves diagnostic accuracy.

Background:

  • Nondiagnostic (ND) thyroid nodule fine-needle aspiration (FNA) rates range from 2% to 20%, leading to repeat procedures and delayed diagnosis.
  • Real-time strain elastography (RTE) is increasingly used for assessing nodule malignancy risk.
  • The utility of RTE in guiding FNA for improved diagnostic yield is less understood.

Purpose of the Study:

  • To investigate if performing FNA concurrently with RTE decreases the rate of ND results.
  • To evaluate the impact of RTE on reducing the need for repeat FNA procedures.

Main Methods:

  • Retrospective review of 221 thyroid nodule FNA cases from 2011-2014.
  • Analysis of nodule characteristics, patient demographics, and specimen adequacy.
  • Comparison of ND rates between cases with and without pre-FNA RTE.

Main Results:

  • RTE was performed in 63.4% of cases (140/221).
  • The ND rate was 16% without RTE versus 10% with RTE.
  • This difference was not statistically significant (P = 0.205).

Conclusions:

  • RTE guidance did not significantly reduce ND FNA rates in this retrospective study.
  • The study design may have influenced the results.
  • Prospective trials with larger sample sizes are recommended to further assess RTE's accuracy in FNA targeting.
