Background:

Histoplasmosis is the most prevalent fungal infection in the United States, caused by *Histoplasma capsulatum*.
Oral manifestations of histoplasmosis are frequently linked to the disseminated form of the disease.
Early and accurate diagnosis of oral fungal infections is crucial for effective patient management.

Observation:

A 44-year-old female presented with a two-week history of a painful oral ulcer.
Clinical examination identified an irregular ulcer on the buccal and lingual gingiva of the anterior mandible.
Biopsy and microscopic evaluation were performed for definitive diagnosis.

Findings:

Microscopic analysis confirmed the presence of histoplasmosis.
The oral lesion presented as an ulcerative type, consistent with disseminated fungal infection.
Histologic findings aid in differentiating oral histoplasmosis from other oral pathologies.

Implications:

This case highlights the importance of considering fungal infections in the differential diagnosis of persistent oral ulcerative lesions.
Recognizing oral histoplasmosis is vital for identifying disseminated disease and initiating appropriate antifungal therapy.
Dentists and clinicians should be aware of the varied presentations of histoplasmosis in the oral cavity.
