Crohns disease can cause multifocal choroiditis and vision loss. Prompt treatment with corticosteroids can resolve these inflammatory eye lesions, even without active gastrointestinal symptoms.
Area of Science:
- Ophthalmology
- Gastroenterology
- Inflammatory Diseases
Background:
- Crohns disease is a chronic inflammatory condition.
- Ocular manifestations of Crohns disease are uncommon but can be severe.
- Multifocal choroiditis and serous retinal detachment are potential ocular complications.
Purpose of the Study:
- To describe a case of Crohns disease presenting with acute vision decrease due to multifocal choroiditis and serous retinal detachment.
- To highlight the importance of considering ocular involvement in Crohns disease patients, even without gastrointestinal symptoms.
Main Methods:
- Comprehensive ophthalmological examination.
- Diagnostic fluorescein angiography to evaluate choroidal lesions.
- Treatment with subtenonian corticosteroid injection.
Main Results:
- Fundus examination revealed multifocal, elevated yellowish lesions in the posterior pole.
- Fluorescein angiography showed characteristic hypofluorescence followed by hyperfluorescence.
- Subtenonian corticosteroid injection led to rapid remission of choroidal lesions.
Conclusions:
- Chorioretinal inflammation in Crohns disease may occur independently of disease activity.
- Patients with Crohns disease presenting with posterior segment inflammation require ophthalmological evaluation.
- Periocular corticosteroid injections are effective in managing these inflammatory chorioretinal lesions.

Inflammatory Bowel Disease II: Crohn's Disease
Inflammatory bowel disease, commonly known as IBD, refers to a collection of disorders that lead to persistent inflammation of the gastrointestinal tract. The two types of IBD are ulcerative colitis, which impacts the colon, and Crohn's disease, which can involve any part of the gastrointestinal segment.
