Summary

Adults who underwent hemispherectomy as children retain some visual function, but often experience constricted visual fields. The eye on the same side as the surgery generally shows better vision than the opposite eye.

Background:

  • Hemispherectomy, a surgical procedure involving the removal of one cerebral hemisphere, is typically performed in childhood to treat severe epilepsy or other neurological conditions.
  • Long-term visual outcomes following hemispherectomy are not well-documented, particularly regarding interocular differences in visual function.

Purpose of the Study:

  • To investigate and characterize visual function in adults who underwent hemispherectomy during childhood.
  • To compare visual function between the ipsilateral and contralateral eyes in post-hemispherectomy adults.

Main Methods:

  • A noncomparative case series involving six adult participants who had undergone hemispherectomy in childhood.
  • Comprehensive visual assessments including visual acuity, binocular function, visual field testing, optical coherence tomography (OCT) for retinal nerve fiber layer (RNFL) thickness, and pattern reversal visually evoked potentials (prVEP).

Main Results:

  • All participants exhibited homonymous hemianopia, with constricted visual fields despite some evidence of visual reorganization into the blind hemifield.
  • The contralateral eye demonstrated significantly reduced visual acuity, RNFL thickness, and visual field height compared to the ipsilateral eye.
  • Divergent strabismus was observed in four participants, correlating with greater visual field loss.

Conclusions:

  • Adults who had a hemispherectomy in childhood may experience better visual function in the ipsilateral eye compared to the contralateral eye.
  • Despite some neural plasticity, significant constriction of the functional visual field persists, highlighting the complex long-term visual consequences of hemispherectomy.
