Background:
- Right ventricle (RV) pressure/volume overload stresses the right bundle branch block (RBBB) network, potentially causing conduction delays.
- Prolonged R' wave duration in lead V1 may indicate reduced RV function in RBBB patients.
Purpose of the Study:
- To investigate the association between R' wave duration in lead V1 and RV dysfunction in patients with RBBB.
- To determine if R' wave duration can serve as a non-invasive marker for RV dysfunction.

- Retrospective review of echocardiography and ECG data from RBBB patients (2013-2015).
- Measurement of QRS and R' wave duration from ECGs.
- RV systolic dysfunction defined as RV fractional area change (FAC) < 35%.

- Patients with RV dysfunction (n=123) had significantly prolonged QRS (145.3±19.3ms) and R' duration (102.9±12.0ms) compared to normal RV function (n=241).
- R' duration strongly correlated with RV FAC (r=-0.609), RV systolic pressure (r=0.142), RV dimension (r=0.193), and RV myocardial performance index (r=0.199).
- An R' duration ≥93ms in lead V1 predicted RV dysfunction with 90% sensitivity and 87% specificity.
Conclusions:
- Prolonged R' wave duration in lead V1 is a significant indicator of RV dysfunction.
- This ECG finding suggests RV pressure and/or volume overload in patients with RBBB.
