Summary

Pediatric vertebral artery dissections (VAD) are linked to rotational neck compression. This study found that dynamic compression in the V3 segment may contribute to VAD in children, though other factors may also be involved.

Background:

  • Spontaneous pediatric vertebral artery dissections (VAD) pathogenesis is unclear.
  • VAD commonly affects the V3 segment, related to C1-C2 rotation.
  • Previous understanding of VAD etiology in children is limited.

Purpose of the Study:

  • To investigate the association between VAD and rotational compression in pediatric patients.
  • To explore the role of dynamic compression in the V3 segment of the vertebral artery.
  • To understand the multifactorial nature of VAD in children.

Main Methods:

  • Study included seven boys aged 12 months to 9 years with V3 VAD.
  • Digital catheter angiography with provocative passive bilateral neck rotation was performed.
  • VAD and dynamic compression were assessed in relation to head rotation.

Main Results:

  • Dynamic V3 compression was observed in 6 of 9 dissections, often with contralateral head rotation.
  • Two patients with VAD showed no dynamic compression initially; both later developed new or worsening dissections.
  • Dynamic compression was also noted in two non-dissected vertebral arteries.

Conclusions:

  • Dynamic vertebral artery compression at the V3 level is associated with pediatric VAD.
  • The findings suggest rotational compression may contribute to VAD, but VAD is likely multifactorial.
  • Further research is needed to fully elucidate the causes of pediatric VAD.
