  • A 56-year-old woman presented with a 16 mm ophthalmic artery aneurysm.
  • The microcatheter repeatedly snagged on the distal aneurysm lip, impeding flow diverter placement.
  • A novel approach was needed to overcome this common obstacle.

Findings:

  • A second microguidewire was introduced alongside the primary microcatheter.
  • This guidewire acted as a 'bumper,' covering the distal aneurysm lip.
  • The technique successfully deflected the microcatheter into the distal parent vessel.

Implications:

  • The 'bumper technique' offers a simple solution for challenging microcatheter navigation.
  • This method may improve the success rate of endovascular treatments for complex aneurysms.
  • Further studies are warranted to evaluate the broader applicability of this technique.
