Summary

Phyllodes tumours (PT) of the breast require adequate surgical margins for benign and borderline types. Adjuvant radiotherapy shows a trend toward improved local control for malignant PT, though metastatic disease has a poor prognosis.

Background:

  • Phyllodes tumour (PT) is a rare breast neoplasm (<1%) with variable pathology and behavior.
  • Understanding the clinical course and treatment efficacy is crucial for patient outcomes.

Purpose of the Study:

  • To describe the clinical course of Phyllodes Tumours (PT) of the breast.
  • To define the role of Radiotherapy (RT) in the management of PT.

Main Methods:

  • Retrospective analysis of hospital data for PT patients from 2005-2014.
  • Descriptive statistics were used to analyze patient demographics, tumor characteristics, and treatment outcomes.

Main Results:

  • 92 patients were analyzed; median age was 43 years, with 60% benign, 23% borderline, and 17% malignant PT.
  • Adequate surgical margins (≥1 cm) resulted in no recurrence for benign and borderline PT.
  • Local recurrence occurred in 3/16 malignant PT patients, and 6/16 developed distant metastases.

Conclusions:

  • Surgical resection with margins >1 cm provides excellent local control for benign and borderline PT.
  • Radiotherapy may be beneficial for borderline PT with close or positive margins and shows a trend towards improved local control in malignant PT.
  • Metastatic malignant PT has a poor prognosis, highlighting the need for effective treatment strategies.
