Background:

  • Large tongue defects often require complex reconstruction.
  • Established methods include radial forearm free flap and anterolateral thigh (ALT) flap.
  • The peroneal flap is frequently utilized for soft tissue defects in head and neck reconstruction at our institution.

Purpose of the Study:

  • To evaluate the efficacy and outcomes of peroneal flap reconstruction for tongue defects.
  • To assess postoperative complications and functional results in patients undergoing tongue reconstruction with peroneal flaps.

Main Methods:

  • Retrospective review of 47 patients who underwent tongue reconstruction using peroneal flaps post-tumor resection.
  • Flap harvesting varied (pure septocutaneous or with muscle bulk) based on defect characteristics.
  • Postoperative complications and functional outcomes (speech, diet, cosmetics) were assessed.

Main Results:

  • Low flap failure rate (6% complete, 2.1% partial necrosis).
  • Hemiglossectomy and subtotal glossectomy groups showed trends towards better functional outcomes (diet) compared to total glossectomy, though not all reached statistical significance.
  • No significant difference in cosmetic outcomes across defect size groups.

Conclusions:

  • The peroneal flap demonstrates a feasible and effective option for tongue reconstruction.
  • It offers acceptable postoperative complication rates and functional results.
  • Further research may explore optimizing functional outcomes based on defect extent.
