Background:

  • Obsessive-compulsive disorder (OCD) is a severe psychiatric condition with unclear pathogenesis.
  • Previous studies indicate inconsistent white matter (WM) alterations in adults with OCD.

Purpose of the Study:

  • To investigate white matter (WM) structure in OCD patients.
  • To test the hypothesis that large-scale brain networks are disrupted in OCD.

Main Methods:

  • Diffusion tensor imaging (DTI) was used to scan 24 OCD patients and 23 healthy controls (HCs).
  • Tract-based spatial statistics (TBSS) and fiber tractography were employed to analyze WM differences.

Main Results:

  • OCD patients exhibited lower fractional anisotropy (FA) in the corpus callosum (CC), left anterior corona radiata (ACR), left superior corona radiata (SCR), and left superior longitudinal fasciculus (SLF).
  • Higher radial diffusivity was observed in the genu and body of the CC in OCD patients.
  • Patients with OCD showed reduced mean FA values in SLF fiber bundles and shorter lengths of ACR, SCR, and CC compared to HCs.

Conclusions:

  • This study provides novel evidence of widespread microstructural WM alterations in OCD.
  • Findings suggest OCD may involve abnormalities in a broader network of brain regions than commonly believed.
