Summary

Machine learning models can differentiate generalized anxiety disorder (GAD) and major depression (MD) from healthy individuals using multimodal biobehavioral data. Cortisol and gray matter volume show promise as neurobiological biomarkers for improved diagnostic accuracy.

Purpose of the Study:

  • To differentiate individuals with GAD or MD from healthy controls (case-classification).
  • To distinguish between GAD and MD (disorder-classification) using machine learning.

  • Machine learning, specifically binary support vector machines, was employed.
  • Data included clinical questionnaires, cortisol release, gray matter (GM), and white matter (WM) volumes.
  • A nested leave-one-out cross-validation framework was utilized with a sample of GAD, MD, and healthy subjects.

  • Clinical questionnaires were effective for case-classification (96.40%) but not disorder-classification (56.58%).
  • Imaging data showed moderate success in disorder-classification (GM: 68.05%, WM: 58.27%).
  • Cortisol data achieved 74.60% accuracy for disorder-classification. Combined data yielded 90.10% for case-classification and 67.46% for disorder-classification.

Conclusions:

  • Clinical questionnaires alone are insufficient for differentiating GAD from MD.
  • Cortisol levels and gray matter volume offer incremental value for GAD classification.
  • Neurobiological biomarkers hold potential for improving diagnostic processes in anxiety and depressive disorders.
