Background:

Delayed post-traumatic mucoceles are uncommon clinical presentations.
Orbital extension of sinonasal mucoceles poses significant surgical challenges.

Observation:

A case of delayed post-traumatic ethmoidal mucocele with intra-orbital extension is presented.
Surgical intervention was performed using a uninostril endoscopic endonasal approach (EEA).
The approach provided contralateral access, facilitating a wider lateral view and identification of critical structures like the optic nerve.
