Summary

Recurrent vitreous hemorrhage can stem from congenital retinal arterial loops, which are actually intraretinal, not preretinal. This finding clarifies the terminology and potential causes of such vascular anomalies.

Area of Science:

  • Ophthalmology
  • Retinal Vascular Diseases
  • Congenital Anomalies

Background:

  • Recurrent spontaneous vitreous haemorrhage (VH) is a significant clinical challenge.
  • Congenital retinal arterial loops are a rare cause of ocular vascular events.

Observation:

  • A case of recurrent spontaneous VH was investigated.
  • Optical coherence tomography (OCT) was utilized to visualize the retinal structures.
  • The arterial loop was identified within the retina, not anterior to it.

Findings:

  • The congenital retinal arterial loop was found to be intraretinal, specifically in the superficial nerve fiber layer.
  • The term 'preretinal arterial loop' is inaccurate based on these findings.
  • Preretinal posterior hyaloid tissue was observed attached to the arterial loop.

Implications:

  • This case redefines the anatomical location of congenital retinal arterial loops.
  • The attached hyaloid tissue may induce traction, explaining recurrent VH.
  • Accurate diagnosis is crucial for understanding the pathogenesis of VH in such cases.
