Background:

  • Gastric antral vascular ectasia (GAVE) is a significant cause of gastrointestinal bleeding.
  • The underlying pathophysiology of GAVE, particularly concerning mucosal blood flow, remains poorly understood.

Purpose of the Study:

  • To investigate and compare gastric antral and body mucosal blood flow in patients with GAVE versus a control group.
  • To elucidate the role of mucosal perfusion in the pathogenesis of GAVE.

Main Methods:

  • Laser Doppler flowmetry (LDF) was employed to measure standardized mucosal blood flow rates.
  • Endoscopic LDF probe was used in 27 GAVE patients and 11 controls, assessing both antrum and gastric body.
  • Perfusion units (PU) were calculated to quantify blood flow.

Main Results:

  • Antral mucosal blood flow in GAVE patients (115.5 PU) was not significantly different from controls (123.7 PU), despite GAVE's hyperemic appearance.
  • A statistically significant gradient in blood flow was observed between the gastric body and antrum in GAVE patients (p < 0.001), which was absent in controls.

Conclusions:

  • Antral mucosal blood flow is not elevated in GAVE, challenging assumptions based on its visual characteristics.
  • A subtle but significant gradient between gastric body and antral blood flow in GAVE patients warrants further investigation into its pathophysiological implications.
