The office anesthesia record

The Anesthesia Record Committee of the Massachusetts Dental Society of Anesthesiology has developed a form to record vital signs and agents used in control of pain in the practice of dentistry. This anesthesia record is simple to use, provides information useful for subsequent dental-anesthetic treatment, and provides adequate written documentation to place the dentist in a legally defensible position.
