    Summary

    Immediate, 6-week, and 4-month delayed single-tooth implant placements showed similar failure and complication rates. However, immediate and 6-week delayed implants offered superior aesthetic outcomes compared to 4-month delayed placements.

    Background:

    • Dental implant placement timing after tooth extraction is crucial for clinical outcomes.
    • Comparing immediate, early, and delayed implant placement strategies is essential for optimizing patient treatment.

    Purpose of the Study:

    • To compare clinical outcomes of single implants placed immediately, 6 weeks (immediate-delayed), or 4 months (delayed) after tooth extraction.

    Main Methods:

    • 210 patients received single implants in one of three groups: immediate, immediate-delayed (6 weeks), or delayed (4 months).
    • Socket grafting was performed when necessary. Implant stability was assessed by insertion torque.
    • Outcomes included implant/crown failures, complications, marginal bone loss, aesthetic scores (PES), and patient satisfaction, followed for 1 year post-loading.

    Main Results:

    • Implant failure rates were 6% (immediate), 6.2% (immediate-delayed), and 1.6% (delayed), with no statistically significant difference.
    • Complication rates were similar across groups (6, 6, and 4 implants, respectively).
    • Mean marginal bone loss was comparable, but aesthetic scores were significantly higher for immediate and immediate-delayed implants.

    Conclusions:

    • No significant differences in implant failures, complications, or patient satisfaction were found between immediate, immediate-delayed, and delayed placement.
    • Immediate and immediate-delayed implant placements demonstrated superior aesthetic results compared to delayed placements.
    • Bone level changes were similar across all timing protocols.
