Background:

Aquagenic papulotranslucent acrokeratoderma (APTA) is a rare skin condition characterized by transient white papules and plaques upon water exposure.
While initially described as hereditary, acquired forms have been linked to cystic fibrosis, drug exposure, or idiopathic causes.

Observation:

A 24-year-old male presented with acquired APTA since infancy.
The patient had no family history of the condition, no prior drug exposure, and no personal or family history of cystic fibrosis.

Findings:

Conventional treatments including urea cream, calcipotriene ointment, vitamin E cream, and clobetasol ointment were ineffective.
This case represents a unique presentation of acquired APTA with a long-standing history and lack of typical associated factors.

Implications:

The case highlights the diverse etiology of acquired APTA, extending beyond known associations.
Botulinum toxin is being considered as a novel therapeutic option for refractory cases of APTA.
Further research into the pathophysiology and treatment of acquired APTA is warranted.
