Summary

Visual hallucinations occur in 16.7% of acute stroke patients and are often self-limited. Occipital lobe lesions and sleep disturbances increase the risk of experiencing these hallucinations following a stroke.

Background:

  • Visual hallucinations in acute stroke patients are understudied.
  • Understanding their incidence, causes, and links to stroke location is crucial.

Purpose of the Study:

  • To investigate the incidence, features, and associations of visual hallucinations in acute stroke.
  • To determine the relationship between visual hallucinations and lesion topography, sleep disturbances, and patient demographics.

Main Methods:

  • Prospective study of 77 acute stroke patients within 24 hours of symptom onset.
  • Exclusion of patients with prior psychosis or cognitive impairment.
  • Assessment of visual hallucinations and sleep using questionnaires; neuroimaging for lesion location and leukoaraiosis; neuropsychological testing and EEG in subsets.

Main Results:

  • 16.7% of patients experienced visual hallucinations, which were typically complex, black and white, and self-limited.
  • Hallucinations were not influenced by age, sex, or cognitive performance.
  • Significant associations were found between visual hallucinations and occipital cortex lesions (P=0.04) and sleep disturbances (P=0.041 and P=0.03).

Conclusions:

  • Visual hallucinations are a relatively common and self-limiting symptom in acute stroke.
  • Patients with occipital lesions and concurrent sleep disturbances are at higher risk.
