  • Blood cultures identified B. cereus.
  • Echocardiography revealed aortic valve vegetations.

Findings:

  • The patient had no typical risk factors for B. cereus endocarditis.
  • A six-week course of rifampicin and levofloxacin achieved complete remission.
  • The patient did not require valve replacement surgery.

Implications:

  • B. cereus should not be dismissed as a mere blood culture contaminant.
  • Successful treatment is possible even in patients without risk factors.
  • Antibiotic resistance in B. cereus endocarditis necessitates careful treatment planning.
