Summary

The study found that imaging criteria for primary-progressive aphasia (PPA) are highly sensitive and specific for the semantic variant but less so for nonfluent and logopenic variants. Interrater reliability for diagnosing PPA subtypes using MRI was also found to be low.

Background:

  • Primary-progressive aphasia (PPA) is a heterogeneous neurodegenerative disorder.
  • Current classification recognizes nonfluent, semantic, and logopenic variants.
  • Variant-specific patterns of brain atrophy are recommended for diagnosis.

Purpose of the Study:

  • To evaluate the sensitivity and specificity of proposed MRI-based imaging criteria for PPA variants.
  • To assess the reliability of these criteria among neuroradiologists.

Main Methods:

  • Analysis of MRI scans from 51 PPA patients, 25 Alzheimer's disease patients, and 26 controls.
  • Voxel-based morphometry for group-level atrophy pattern confirmation.
  • Individual MRI reports by three senior neuroradiologists.

Main Results:

  • High sensitivity (92%) and specificity (93%) for semantic-variant PPA.
  • Lower sensitivity for nonfluent (21%) and logopenic (43%) variants, but high specificity.
  • Intrarater agreement was good (mean κ=0.63), but interrater agreement was poor (mean κ=0.41).

Conclusions:

  • Proposed imaging criteria are effective for semantic-variant PPA at the individual level.
  • MRI criteria show limited sensitivity for nonfluent and logopenic PPA variants.
  • Low interrater reliability highlights challenges in consistent diagnosis of PPA subtypes.
