Background:

The Milestones Project mandates a competency-based evaluation framework for all ACGME-accredited programs.
Despite widespread implementation, the Milestones' impact and perceptions have not been extensively studied.

Purpose of the Study:

To investigate general surgery program directors' perceptions of the Milestones Project.
To assess the perceived benefits and drawbacks of the Milestones evaluation system.

Main Methods:

A national survey was administered to general surgery program directors.
Data were collected between January and March 2016 from 132 respondents (60% response rate).

Main Results:

Most PDs perceived value in education (55%) and resident performance evaluation (58%).
Positive feedback included objective feedback (55%) and deficiency identification (58%).
Concerns were raised regarding time commitment (25%) and program discrimination (21%).

Conclusions:

Surgical PDs reported benefits in education and objective evaluation using the Milestones.
The Milestones were generally viewed as an improvement over previous systems.
PDs recommended customization for programmatic differences in future iterations.
