Standing knee alignment after total knee arthroplasty (TKA) poorly predicts dynamic knee function during gait. This suggests static alignment may have limited impact on long-term implant success and wear.

  • To assess the relationship between standing Hip-Knee-Ankle (HKA) alignment and dynamic HKA during gait.

  • Prospective study of 35 patients undergoing posterior-stabilized TKA for osteoarthritis.
  • Post-operative long-leg radiographs to classify standing HKA (neutral, varus, valgus).

  • Standing HKA showed moderate correlation with peak dynamic varus and adduction moments.

  • Standing HKA is of limited value in predicting dynamic knee behavior during gait.
  • This finding may explain the weaker-than-expected link between static alignment and long-term TKA implant fixation and wear.
