Area of Science:

  • Gastroenterology
  • Surgical Pathology

Background:

  • Gallbladder perforation is a severe complication associated with significant morbidity and mortality.
  • Delayed diagnosis is a primary factor contributing to poor patient outcomes.
  • Diagnosis frequently occurs intraoperatively, highlighting challenges in pre-surgical identification.

Purpose of the Study:

  • To emphasize the importance of considering gallbladder perforation in differential diagnoses.
  • To highlight the diagnostic challenges and implications of delayed recognition.
  • To underscore the need for early suspicion in peritonitis cases without clear etiology.

Main Methods:

  • Review of clinical presentations and diagnostic pathways for gallbladder perforation.
  • Analysis of factors contributing to delayed diagnosis.
  • Comparative assessment against other causes of peritonitis.

Main Results:

  • Gallbladder perforation is frequently identified only at the time of surgery.
  • Late diagnosis is strongly correlated with increased morbidity and mortality.
  • It remains a rare but critical consideration in undiagnosed peritonitis.

Conclusions:

  • Early consideration of gallbladder perforation is crucial for timely intervention.
  • Improved diagnostic strategies are needed to facilitate pre-operative identification.
  • Ruling out gallbladder perforation is essential in unexplained peritonitis to improve patient outcomes.
