Summary

Pressurized Intraperitoneal Aerosol Chemotherapy (PIPAC) is a novel treatment for peritoneal carcinomatosis. Implementing PIPAC with a standardized approach and safety checklist allows for safe clinical introduction with a minimal learning curve.

Background:

Pressurized Intraperitoneal Aerosol Chemotherapy (PIPAC) offers a new treatment modality for peritoneal carcinomatosis.
Safe implementation relies on meticulous patient selection, robust safety protocols, and precise surgical techniques.

Purpose of the Study:

To detail the essential elements for the safe implementation of PIPAC.
To analyze practical and surgical aspects during the initial phase of PIPAC introduction.

Main Methods:

Analysis of 127 consecutive PIPAC procedures over 20 months.
Emphasis on technique modifications and safety measures, utilizing a dedicated checklist.
Documentation of surgical difficulty using a visual analogue scale (VAS).

Main Results:

127 PIPAC procedures performed in 58 patients; 81% had prior laparotomies.
Low complication rate: 2% non-access (in prior HIPEC patients), one bowel lesion with open Hasson technique.
No cytostatic leaks or procedure cessations; low reported difficulty (VAS 3 ± 2.4).

Conclusions:

PIPAC can be safely integrated into clinical practice.
A standardized surgical approach and a dedicated safety checklist are key to a successful introduction.
Minimal learning curve observed for PIPAC implementation.
