This study examined the Public Action Organizational Contract (Coap) implementation and its effect on state health agendas. States showed varied engagement patterns, with some fragmented, some selective, and others consistently aligned with national directives.
Area of Science:
- Public Health
- Health Policy
- Health Management
Background:
- The Public Action Organizational Contract (Coap) is a strategy to guide SUS regionalization.
- Decree 7.508/11 outlines the normative framework for Coap implementation.
- Understanding state-level institutional reactions to Coap is crucial for effective regionalization.
Purpose of the Study:
- To analyze the Coap implementation process and its impact on state health agendas.
- To compare institutional reactions of Brazilian states to the Coap strategy.
- To identify different patterns of state engagement with the Coap agenda.
Main Methods:
- Data collected via a 35-question survey (mixed-methods) and extensive documentary research.
- Study conducted in 2013-2014 across all Brazilian states (excluding the Federal District).
- Analysis based on a normative baseline of eight strategic political-institutional moments for Coap implementation.
Main Results:
- Three distinct state engagement patterns emerged: fragmented (6 states), selective (12 states), and regular (10 states).
- States exhibited varied distances from the national agenda set by Decree 7.508/11.
- Selective engagement often involved strengthening existing regionalization processes.
Conclusions:
- State responses to Coap implementation are heterogeneous, reflecting diverse institutional capacities and priorities.
- The findings highlight the need for tailored strategies to support state-level SUS regionalization.
- Effective Coap implementation requires understanding and addressing these differentiated state dynamics.
