Background:

  • Silicone oil (SO) is a common intravitreal tamponade for rhegmatogenous retinal detachment (RRD) with proliferative vitreoretinopathy or large tears.
  • Typically retained for 3+ months, SO is generally well-tolerated despite known complications.
  • Some cases report unexplained central vision loss post-SO use or removal.

Purpose of the Study:

  • To investigate the causes of visual loss occurring after intraocular silicone oil (SO) use or removal.
  • To analyze visual outcomes in patients undergoing vitrectomy for RRD.

Main Methods:

  • Retrospective case series of 12 patients with macula-on RRD experiencing visual loss post-SO.
  • Evaluations included fundus fluorescein angiography, OCT, perimetry, and electrophysiology.
  • Retro-oil fluid electrolyte levels were compared to vitreous humor in other vitrectomy cases.

Main Results:

  • A decrease of at least 2 Snellen lines in visual acuity was observed.
  • No macular detachment was present in any of the reviewed cases.
  • Most patients showed unremarkable findings on angiography and OCT; electrolyte levels were similar between groups.

Conclusions:

  • Visual loss is a potential complication associated with intraocular silicone oil use or removal.
  • The etiology of this vision loss remains largely unknown and requires further research.
