Summary

A woman with metastatic osteosarcoma unexpectedly tested positive for pregnancy. Elevated beta human chorionic gonadotropin (βhCG) levels were linked to the tumor, not pregnancy, and normalized with treatment.

Area of Science:

  • Oncology
  • Reproductive Endocrinology
  • Pathology

Background:

  • Osteosarcoma is a primary bone cancer, typically affecting children and young adults.
  • Paraneoplastic syndromes occur when cancer triggers a systemic effect, sometimes involving hormone production.
  • Ectopic beta human chorionic gonadotropin (βhCG) secretion is a rare paraneoplastic phenomenon.

Observation:

  • A 35-year-old woman presented with right knee swelling due to metastatic osteosarcoma.
  • Routine pregnancy tests were positive, with rising serum βhCG levels.
  • Transvaginal ultrasound revealed no gestational sac, ruling out a clinical pregnancy.

Findings:

  • Radiological and pathological evaluations confirmed metastatic osteosarcoma.
  • The elevated βhCG levels correlated with osteosarcoma activity.
  • βhCG levels normalized following chemotherapy, indicating tumor response.

Implications:

  • This case highlights a rare instance of βhCG production by osteosarcoma.
  • Monitoring βhCG levels may serve as a biomarker for osteosarcoma activity and treatment response.
  • Further research is needed to understand the prognostic significance of βhCG in diverse tumor types.
