    Summary

    A young soldier experienced chronic low back pain, initially attributed to common causes. Advanced imaging revealed a rare epithelioid hemangioma in the L1 vertebra, highlighting the importance of thorough investigation for persistent pain.

    Background:

    • A 24-year-old male soldier presented with a 2-year history of insidious-onset, progressive low back pain.
    • Initial lumbar radiographs were noncontributory, and the patient was referred for physical therapy.

    Observation:

    • Magnetic resonance imaging (MRI) revealed a complex mass and cortical fracturing in the L1 vertebral body.
    • Computed tomography (CT) imaging showed mixed sclerotic and lytic lesions at L1.

    Findings:

    • Biopsy confirmed the diagnosis of epithelioid hemangioma, a rare vascular tumor, within the L1 vertebra.

    Implications:

    • This case underscores the importance of considering rare pathologies in the differential diagnosis of persistent low back pain, even in young individuals.
