Refractory Cutaneous IgA Vasculitis Treated with Omega-3 Fatty Acids

Omega-3 fatty acids (O3FA) show promise in managing cutaneous IgA vasculitis (IgAV). This case report suggests O3FA can help control IgAV flares, potentially reducing the need for other medications.

Background:
- Cutaneous IgA vasculitis (IgAV) is an autoimmune condition often associated with IgA nephropathy (IgAN).
- Omega-3 fatty acids (O3FA) have demonstrated therapeutic effects in IgAN but their role in IgAV remains less explored.
Observation:
- A 47-year-old female with a 24-year history of IgAV and IgAN experienced persistent disease flares.
- Conventional treatments including prednisone and various immunosuppressants failed to achieve sustained disease control.
- Introduction of O3FA (eicosapentaenoic acid and docosahexaenoic acid) led to successful prednisone withdrawal and disease remission.
Findings:
- High-dose O3FA therapy enabled the cessation of prednisone in a patient with refractory IgAV.
- Disease flares recurred upon O3FA dose reduction, highlighting the importance of consistent O3FA intake for disease management.
Implications:
- Omega-3 fatty acids may represent a novel therapeutic option for managing cutaneous IgA vasculitis.
- Further research is warranted to confirm the efficacy and optimal dosing of O3FA in IgAV patients.
