Summary

Measuring cytomegalovirus (CMV)-specific cell-mediated immunity (CMI) safely guides early antiviral discontinuation in transplant patients. This approach reduces recurrence compared to standard care, demonstrating feasibility for clinical practice.

Background:

  • Cell-mediated immunity (CMI) predicts cytomegalovirus (CMV) events but isn't routinely used.
  • Lack of interventional studies hinders CMI adoption in clinical practice.

Purpose of the Study:

  • To demonstrate the safety and feasibility of early antiviral discontinuation based on real-time CMV-specific CMI.
  • To guide management of CMV viremia in transplant patients using CMI assessment.

Main Methods:

  • Enrolled transplant patients with CMV viremia requiring antiviral therapy.
  • Assessed CD8 T cell responses using the Quantiferon-CMV assay for real-time CMI measurement.
  • Guided antiviral management based on CMV-specific CMI results.

Main Results:

  • 14/27 (51.9%) patients with positive CMV-CMI had antivirals discontinued early.
  • Only one patient with early discontinuation experienced recurrence.
  • 69.2% of CMI-negative patients experienced recurrence despite prolonged antivirals (p = 0.001).

Conclusions:

  • This study is the first to show feasibility and safety of real-time CMV-specific CMI assessment.
  • CMI-guided management can effectively reduce CMV recurrence in transplant patients.
  • Real-time CMI assessment can guide changes in CMV infection management.
