Area of Science:

  • Medical Physics
  • Radiation Oncology
  • Brachytherapy

Background:

  • Brachytherapy utilizes applicators for precise radiation delivery.
  • CT-compatible applicators allow for accurate treatment planning and dose verification.
  • Understanding applicator shielding is crucial for optimizing dose to target and organs at risk.

Purpose of the Study:

  • To characterize a new CT-compatible Low-Dose-Rate (LDR) Tandem and Ovoid (T&O) applicator.
  • To evaluate source attenuation through asymmetric gold shielding using 3D dosimetry.
  • To compare 3D dosimetry measurements with analytical calculations and diode measurements.

Main Methods:

  • Utilized a PRESAGE dosimeter and the Duke Large field of view Optical CT-Scanner (DLOS) for 3D dosimetry.
  • Measured dose transmission through gold shielding at various angles.
  • Reconstructed 3D dose distribution and applied it to a CT-based T&O implant model.

Main Results:

  • Dose transmission through gold shielding ranged from approximately 51% to 87%.
  • The T&O applicator's shielding decreased the average Point A dose by ~4%.
  • D2cc to the bladder, rectum, sigmoid, and bowel decreased by 2%, 15%, 2%, and 7%, respectively.

Conclusions:

  • 3D dosimetry provided unique insights into the applicator's shielding characteristics.
  • The technique is applicable for commissioning other brachytherapy applicators.
  • Accurate dosimetry is essential for safe and effective brachytherapy treatment.
