Background:

  • Visual field variability can complicate the detection of glaucoma progression.
  • Overlap exists between neurocognitive disorders and glaucoma in older adults, but their association with visual field variability is understudied.

Purpose of the Study:

  • To investigate the relationship between global neurocognitive impairment and visual field variability in individuals with glaucoma or glaucoma suspect status.

Main Methods:

  • A prospective observational cohort study included 115 patients (211 eyes) followed for a mean of 2.5 years.
  • Standard automated perimetry (SAP) assessed visual fields, while the Montreal Cognitive Assessment (MoCA) evaluated cognitive ability.
  • Visual field variability was quantified using the standard deviation of residuals from linear regression of SAP mean deviation (MD) over time.

Main Results:

  • A statistically significant association was found between changes in MoCA scores and visual field variability.
  • In multivariable analysis, a 5-point decline in MoCA score correlated with a 0.23 dB increase in the standard deviation of SAP MD residuals (95% CI, 0.11-0.35; P < .001).

Conclusions:

  • Cognitive decline is associated with increased visual field variability in glaucoma patients.
  • Screening for and monitoring cognitive dysfunction may be crucial for assessing visual field progression in glaucoma.
