Overview of Flail Chest
Flail chest is a severe and potentially life-threatening condition characterized by the fracture of three or more adjacent ribs in multiple places. It is most commonly caused by direct impacts and trauma, such as motor vehicle accidents or injuries from a steering wheel impact. It can also occur due to falls in elderly individuals with osteoporosis, or assaults involving sharp objects.

    Background:

    • Tarlov cysts, or perineural cysts, are meningeal dilations.
    • These cysts typically occur in the sacrum, involving nerve roots.
    • While often asymptomatic, they can cause radiculopathy, paresthesias, and dysfunction.

    Observation:

    • A 38-year-old female presented with severe low back and buttock pain radiating down her leg.
    • Imaging revealed a large sacral Tarlov cyst with an associated insufficiency fracture.
    • The patient exhibited an antalgic gait but otherwise had a normal neurological exam.

    Findings:

    • Surgical exploration identified the cyst and associated nerve roots.
    • Significant bone erosion and sacral insufficiency fractures were noted.
    • The cyst was fenestrated, and sacropelvic fixation with L5 fusion was performed due to extensive bone damage.

    Implications:

    • Sacral Tarlov cysts can cause substantial bone erosion and insufficiency fractures.
    • Treatment may necessitate cyst fenestration and complex sacropelvic fixation.
    • This case highlights a rare but significant presentation of Tarlov cysts.
