    Background:

    • Paget's disease of bone is a chronic disorder characterized by abnormal bone remodeling.

    Observation:

    • A 68-year-old male patient with Paget's disease of bone was treated with calcitonin.
    • The patient developed a sarcoma in the sacroiliac region during calcitonin therapy.
    • No evidence of sarcoma was present prior to the initiation of calcitonin treatment.

    Findings:

    • The development of sacroiliac sarcoma occurred during calcitonin treatment for Paget's disease of bone.
    • Calcitonin therapy did not appear to influence the progression of the sarcoma.

    Implications:

    • This case highlights a potential association between calcitonin treatment and sarcoma development in Paget's disease of bone patients.
    • Further research is needed to investigate the potential link between calcitonin and sarcoma in bone disorders.
    • Clinicians should remain vigilant for secondary malignancies in patients undergoing long-term treatment for Paget's disease of bone.
