Summary

Retinal sensitivity (Se) is a better indicator of visual impairment in dome-shaped macula (DSM) than visual acuity. Foveal subretinal fluid and increased bulge height significantly impact Se in specific DSM types.

Background:

  • Dome-shaped macula (DSM) is an anatomical variation that can affect visual function.
  • Assessing the functional impact of DSM is crucial for patient management.

Purpose of the Study:

  • To evaluate retinal sensitivity (Se) in patients with dome-shaped macula (DSM) using microperimetry.
  • To correlate microperimetry findings with specific spectral domain optical coherence tomography (SD-OCT) features in DSM.

Main Methods:

  • A prospective, cross-sectional study involving 53 eyes of 29 patients with DSM.
  • Measurements included best-corrected visual acuity, microperimetry for Se, and SD-OCT for DSM pattern, bulge height, and retinal/choroidal thickness.

Main Results:

  • Foveal subretinal fluid was present in 54.7% of cases and correlated with bulge height and reduced Se, but not visual acuity.
  • Se was significantly impaired in vertically-oriented DSM (V-DSM) and symmetric DSM (S-DSM) with subretinal fluid, but not in horizontally-oriented DSM (H-DSM).
  • In H-DSM, subretinal fluid was associated with increased inferior choroidal thickness.

Conclusions:

  • Retinal sensitivity (Se) assessment is more sensitive than best-corrected visual acuity for detecting central visual impairment in DSM.
  • Specific SD-OCT features like foveal subretinal fluid and higher bulge height are key indicators of functional deficits in DSM.
