Summary

Myoglobin levels do not predict local toxicity in patients undergoing isolated limb perfusion (ILP) for melanoma metastases. This study found no significant correlation between myoglobin measurements and the severity of local side effects after ILP.

Background:

  • Isolated limb perfusion (ILP) treats in-transit melanoma metastases and advanced sarcomas by delivering high-dose chemotherapy to an extremity.
  • While ILP minimizes systemic toxicity, it can cause local adverse effects like edema, blistering, nerve damage, and compartment syndrome.
  • Myoglobin has been explored as a potential biomarker to screen for severe local toxicity following ILP.

Purpose of the Study:

  • To determine if myoglobin levels can predict local toxicity in patients with melanoma in-transit metastases undergoing ILP.
  • To assess the correlation between post-perfusion myoglobin measurements and the severity of local adverse events.

Main Methods:

  • A retrospective analysis of 193 patients treated with ILP for in-transit metastases between 2001 and 2015.
  • Myoglobin levels were measured on post-operative day 0 (POD0) and POD1-5.
  • Local toxicity was graded using the Wieberdink scale (mild, moderate, severe) and compared with myoglobin levels, sex, tumor size, and number.

Main Results:

  • No statistically significant difference in serum myoglobin levels was observed during the first five post-operative days across different local toxicity severity groups (mild, moderate, severe).
  • No significant differences in sex, tumor size, or tumor number were found between toxicity groups.

Conclusions:

  • Myoglobin measurements taken within the first five post-operative days are not predictive of local toxicity in patients with melanoma in-transit metastases treated with ILP.
  • The utility of myoglobin as a predictive biomarker for ILP-induced local toxicity in this patient population is not supported by these findings.
