Bullous morphoea, a rare localized scleroderma variant, was found in 1.4% of cases. This study highlights potential roles for lymphatic vessels and skin trauma in its development.

Area of Science:

  • Dermatology
  • Pathology

Background:

  • Bullous morphoea is a rare variant of localized scleroderma.
  • Its pathogenesis remains a subject of discussion.

Observation:

  • A retrospective review of 137 morphoea cases (2005-2015) identified 2 bullous morphoea cases.
  • This represents 1.4% of morphoea cases, lower than previous reports (7.5%).

Findings:

  • Bullous morphoea cases were successfully treated with methotrexate.
  • Histopathology revealed basal keratinocyte stretching and altered lymphatic vessels (collapsed or dilated).

Implications:

  • Findings suggest a role for lymphatic dysfunction in bullous morphoea development.
  • Excessive skin trauma and friction may also contribute to bullous lesion formation.
