Low blood levels of the thyroid hormones — triiodothyronine (T3) and thyroxine (T4) — signal the hypothalamus to release the thyrotropin-releasing hormone (TRH). TRH then reaches the pituitary gland and stimulates the release of thyroid-stimulating hormone(TSH) into the bloodstream.

Summary

A 70% decrease in parathyroid hormone (PTH) after thyroidectomy indicates a high risk for symptomatic hypocalcemia (SxH). Patients with a PTH decrease of 70% or less can be discharged on postoperative day 1.

Background:

  • Symptomatic hypocalcemia (SxH) is a common complication after thyroidectomy.
  • Optimal management strategies for post-thyroidectomy SxH remain unclear.

Purpose of the Study:

  • To establish a standardized protocol for managing post-thyroidectomy SxH.
  • To utilize relative parathyroid hormone (PTH) change and postoperative day 1 (POD1) PTH levels for risk stratification.

Main Methods:

  • Retrospective analysis of 744 patients undergoing thyroidectomy.
  • Measurement of pre-surgery and POD1 PTH levels.
  • Recording of SxH signs, treatment, and follow-up for 1 month, then 3 months.

Main Results:

  • A relative PTH decrease >70% was a significant predictor of SxH (AUC=0.754).
  • Patients with >70% PTH decrease experienced longer symptom relief duration.
  • Calcitriol treatment was more common in patients with POD1 PTH <1 pmol/L, but showed no difference in outcomes.

Conclusions:

  • A relative PTH decrease >70% is a key risk factor for post-thyroidectomy SxH.
  • Patients with ≤70% PTH decrease are suitable for POD1 discharge.
  • Extended hospitalization is recommended for patients with >70% PTH decrease, especially if POD1 PTH <1 pmol/L, requiring calcitriol.
