Summary

Congenital giant nevi, originating from neuroectodermal cells, can develop into diverse tumors. This case highlights mixed melanocytic and neural differentiation within a giant nevus, leading to a malignant melanoma.

Area of Science:

  • Dermatology and Oncology
  • Developmental Biology
  • Neurocristopathies

Background:

  • Congenital giant nevi are rare, neuroectodermal-derived birthmarks predominantly composed of melanocytes.
  • These nevi can exhibit variable clinical behavior and harbor neoplasms with heterogeneous morphology.
  • Understanding the developmental potential of neural crest derivatives is crucial for managing these complex lesions.

Observation:

  • A female infant presented with multiple pigmented nevi, including a bathing trunk giant hairy nevus.
  • A large pedunculated mass developed within the giant nevus, involving the sacral region, buttocks, perineum, and left thigh.
  • Histological examination revealed mixtures of melanocytic (nevus) and neural supportive (Schwannian) cell differentiation within the neoplasm.

Findings:

Related Experiment Videos

  • The neural differentiation exhibited a neurofibromatous appearance with pseudomeissnerian corpuscles in the dermis and subcutaneous tissue.
  • At 16 months, the child developed a malignant melanoma within the flank region of the congenital giant nevus.
  • This case demonstrates the morphologic pluripotentiality of primitive neural crest derivatives within congenital nevi.

Implications:

  • Congenital giant nevi should be considered neuroectodermal neoplasms due to their complex cellular origins.
  • Subclassification based on differentiating histologic characteristics is essential for accurate prognosis and management.
  • Further research into the developmental pathways of neural crest cells in congenital nevi is warranted.
