Cognitive Reserve (CR) protects against cognitive decline in cirrhosis patients. Higher CR scores correlated with better cognitive function and a reduced mismatch between cognitive and neurophysiological assessments of hepatic encephalopathy (HE).

- Eighty-two outpatient patients with cirrhosis but without overt HE were recruited.
- Cognitive function was assessed using the Psychometric Hepatic Encephalopathy Score (PHES).
- Neurophysiological assessment was performed using electroencephalogram (EEG) spectral analysis. Cognitive Reserve Index (CRI) was measured using the CRI questionnaire (CRIq).
Main Results:
- 28% of patients showed altered PHES and 41% had abnormal EEG findings.
- Patients with preserved PHES had significantly higher CRI scores compared to those with altered PHES (p < 0.01).
- The PHES, but not EEG, was significantly related to CRI (r = 0.35, p < 0.01). The cognitive-neurophysiological mismatch also correlated with CRI (r = 0.36, p < 0.005).
Conclusions:
- Cognitive Reserve acts as a resilience factor against cognitive dysfunction in cirrhosis patients.
- The CRI questionnaire (CRIq) provides an easily measurable assessment of CR in this population.
- CR influences the manifestation and detection of cognitive impairment in cirrhosis, particularly in relation to HE assessment.
