Mitral valve repair offers better survival and fewer thromboembolic events than replacement in hypertrophic obstructive cardiomyopathy patients undergoing myectomy. Both surgical strategies effectively reduce left ventricular outflow tract obstruction.
Area of Science:
- Cardiovascular Surgery
- Cardiac Surgery
- Thoracic Surgery
Background:
- Hypertrophic obstructive cardiomyopathy (HOCM) presents challenges in surgical management, particularly regarding concomitant mitral valve disease.
- The optimal surgical approach for severe mitral regurgitation during myectomy for HOCM is debated.
Purpose of the Study:
- To compare the outcomes of mitral valve replacement versus mitral valve repair in patients with HOCM and severe mitral regurgitation undergoing extended myectomy.
Main Methods:
- A randomized controlled trial involving 88 patients with HOCM and severe mitral regurgitation between 2010 and 2013.
- Patients were assigned to either mitral valve replacement or repair, in addition to extended myectomy.
Main Results:
- Both groups showed significant reduction in left ventricular outflow tract gradients post-surgery.
- Mitral valve repair was associated with significantly better overall survival (96.7% vs 87.2%) and a lower rate of thromboembolic events (100% vs 91.2%) at 2-year follow-up.
- One early death occurred in the replacement group; three patients in the repair group required conversion to replacement due to repair failure.
Conclusions:
- Both mitral valve repair and replacement are effective surgical options for HOCM patients with severe mitral regurgitation undergoing myectomy.
- Mitral valve repair demonstrates superior long-term outcomes regarding survival and thromboembolic event prevention.
