    Endoscopic sphincterotomy for bile duct stones can lead to persistent biliary abnormalities, including reflux and elevated liver enzymes, despite symptom improvement. Long-term consequences of these changes remain uncertain.

    Background:

    • Endoscopic sphincterotomy (ES) is a common procedure for bile duct stone removal.
    • Long-term outcomes and potential complications of ES require further investigation.

    Purpose of the Study:

    • To evaluate persistent biliary abnormalities after successful ES for bile duct stones.
    • To assess the association between biliary reflux and clinical symptoms or liver histology.

    Main Methods:

    • Retrospective review of 25 patients post-ES for bile duct stones.
    • Assessment of symptoms, liver function tests, and biliary gas via imaging.
    • Aspiration liver biopsy in patients with biliary reflux.

    Main Results:

    • All patients reported symptomatic improvement post-ES.
    • 50% of patients showed biliary gas, indicating duodenal reflux.
    • Biliary reflux was linked to mild abdominal pain and portal tract inflammation on biopsy.
