Summary

Endophthalmitis after open globe injury, particularly with foreign bodies or delayed repair, presents challenges. Forty percent of affected eyes achieved final vision of 20/200 or better.

Background:

Purpose of the Study:

  • To characterize the demographics, clinical features, management strategies, and visual outcomes of endophthalmitis following open globe injury.
  • To identify factors associated with concurrent versus post-operative open globe repair endophthalmitis.

Main Methods:

  • Retrospective cohort analysis of patients diagnosed with endophthalmitis after open globe injury between 1997 and 2015.
  • Data collected included patient demographics, injury characteristics, microbiological findings, treatment, and visual acuity outcomes.

Main Results:

  • Twenty-six male patients (mean age 37) were analyzed.
  • Staphylococcus species were the most common pathogens (44%).
  • Concurrent endophthalmitis was associated with intraocular foreign bodies and delayed primary open globe repair (OGR).
  • Twenty-nine percent of post-OGR endophthalmitis cases had wound leaks.
  • Final best-corrected visual acuity (BCVA) ≥20/200 was achieved in 40% of eyes.

Conclusions:

  • Intraocular foreign bodies and delayed presentation are risk factors for concurrent endophthalmitis.
  • Wound leaks are a complication in nearly a third of post-OGR endophthalmitis cases.
  • Achieving a BCVA of 20/200 or better is possible in 40% of eyes with endophthalmitis after open globe injury.
