Background:

  • Multiple sclerosis (MS) and psoriasis are chronic inflammatory conditions with known epidemiological and biological links.
  • The impact of comorbid psoriasis on the clinical course of multiple sclerosis has not been previously investigated.

Purpose of the Study:

  • To characterize patients with both psoriasis and MS.
  • To determine if the presence of psoriasis influences the progression of multiple sclerosis.

Main Methods:

  • Retrospective case-control study utilizing data from the Sheba Hospital Multiple Sclerosis Center database.
  • Identified MS patients with psoriasis comorbidity and compared their clinical and demographic characteristics to a matched cohort of MS-only patients.
  • Analyzed MS progression outcomes in patients with over 5 years of follow-up.

Main Results:

  • 1.3% of MS patients (45/3456) had comorbid psoriasis, with psoriasis preceding MS in 78% of cases.
  • Patients with psoriasis onset before relapsing-remitting MS (RRMS) exhibited slower disease progression.
  • This slower progression was evidenced by a significantly longer time to a second relapse (P < 0.01) and to significant neurological disability (P < 0.03).

Conclusions:

  • Psoriasis comorbidity, particularly when preceding MS onset, is associated with a slower rate of disability progression in multiple sclerosis patients.
  • This suggests a potential modifying effect of psoriasis on MS pathogenesis or clinical trajectory.
