Summary

Multiple solid organ injuries (SOIs) in blunt abdominal trauma patients correlate with increased morbidity and operative management needs. Pancreatic and hepatic injuries show higher mortality rates in single SOIs.

Background:

  • Solid organ injuries (SOIs) are a significant component of trauma admissions.
  • Understanding the patterns of SOIs is crucial for effective trauma care.

Purpose of the Study:

  • To describe the pattern of solid organ injuries (SOIs).
  • To analyze characteristics, management, and outcomes based on the multiplicity of SOIs.

Main Methods:

  • Retrospective study at a Level 1 trauma center.
  • Included patients with blunt abdominal trauma (2011-2014).
  • Compared data for patients with single versus multiple SOIs.

Main Results:

  • Liver (45%), spleen (30%), and kidney (18%) were most frequently injured.
  • Multiple SOIs occurred in one-fifth of patients, typically involving two organs.
  • Multiple SOIs were associated with higher head injury frequency, injury severity scores, operative management, blood transfusion, sepsis, and longer hospital stays.
  • Overall mortality was 11%, comparable between single and multiple SOI groups.
  • Pancreatic (28.6%) and hepatic (13%) injuries had higher mortality in the single SOI group.

Conclusions:

  • SOIs constitute 10% of trauma admissions in Qatar.
  • While liver injuries are most common, pancreatic injuries carry a higher mortality risk.
  • Multiple SOIs increase morbidity and necessitate more frequent operative interventions.
  • Prospective studies are needed to develop management algorithms based on SOI multiplicity.
