Summary

Bendamustine hydrochloride shows potential in treating relapsed blastic plasmacytoid dendritic cell neoplasm (BPDCN). Further evaluation is warranted for this rare cancer with poor prognosis.

Background:

Blastic plasmacytoid dendritic cell neoplasm (BPDCN) is a rare hematologic malignancy with a poor prognosis.
Optimal treatment strategies for BPDCN remain undefined.
BPDCN was previously classified as CD4+/CD56+ hematodermic malignancies.

Observation:

This study reports on five cases of relapsed BPDCN treated with bendamustine hydrochloride.
All patients were over 50 years old and had advanced disease, including multi-organ involvement and prior intensive therapies.
Four patients were evaluable for treatment response.

Findings:

One patient achieved complete remission and maintained it for seven months.
Two patients did not respond to bendamustine therapy.
One patient died from tumor lysis syndrome following rapid blast clearance.

Implications:

Bendamustine hydrochloride, a bifunctional alkylating and antimetabolite agent, demonstrates a potential role in managing relapsed BPDCN.
Further clinical trials are necessary to establish the efficacy and safety of bendamustine in BPDCN treatment.
This study highlights the need for novel therapeutic approaches for BPDCN.
