Summary

Bendamustine hydrochloride shows potential in treating relapsed blastic plasmacytoid dendritic cell neoplasm (BPDCN). Further evaluation is warranted for this rare cancer with poor prognosis.

Background:

  • Blastic plasmacytoid dendritic cell neoplasm (BPDCN) is a rare hematologic malignancy with a poor prognosis.
  • Optimal treatment strategies for BPDCN remain undefined.
  • BPDCN was previously classified as CD4+/CD56+ hematodermic malignancies.

Observation:

  • This study reports on five cases of relapsed BPDCN treated with bendamustine hydrochloride.
  • All patients were over 50 years old and had advanced disease, including multi-organ involvement and prior intensive therapies.
  • Four patients were evaluable for treatment response.

Findings:

  • One patient achieved complete remission and maintained it for seven months.
  • Two patients did not respond to bendamustine therapy.
  • One patient died from tumor lysis syndrome following rapid blast clearance.

Implications:

  • Bendamustine hydrochloride, a bifunctional alkylating and antimetabolite agent, demonstrates a potential role in managing relapsed BPDCN.
  • Further clinical trials are necessary to establish the efficacy and safety of bendamustine in BPDCN treatment.
  • This study highlights the need for novel therapeutic approaches for BPDCN.
