Persistent ectopic pregnancy: complication of conservative surgery?

    Summary

    Conservative surgery for ectopic pregnancy can lead to persistent trophoblastic growth, requiring further treatment. Postoperative monitoring with serial hCG tests is recommended to detect this complication early.

    Background:

    • Conservative surgical management of unruptured tubal gestation is standard practice.
    • Complete removal of trophoblastic tissue during conservative surgery cannot always be guaranteed.

    Observation:

    • A complication of persistent trophoblastic growth after conservative tubal gestation surgery has been reported.
    • Five cases, including the authors' recent experience, are documented in the literature.
    • This complication may be under-reported in clinical practice.

    Findings:

    • Persistent trophoblastic growth necessitates a second, more radical surgical procedure.
    • Routine serial beta-subunit human chorionic gonadotropin (hCG) surveillance is recommended postoperatively.
    • Patients should be informed about the risk of this complication before conservative surgery.

    Implications:

    • Early detection of persistent trophoblastic growth can be achieved through serial hCG monitoring.
    • Cytotoxic agents may offer a potential management strategy, similar to gestational trophoblastic disease.
    • Informing patients about potential complications enhances informed consent in ectopic pregnancy management.
