Background:

  • Giant cell arteritis (GCA) is a systemic autoimmune vasculitis affecting medium to large arteries.
  • The relationship between GCA and autoimmune thyroid disease remains incompletely understood, with prior studies yielding inconsistent findings.

Purpose of the Study:

  • To investigate the association between giant cell arteritis and thyroid dysfunction, specifically hypothyroidism and hyperthyroidism.
  • To determine if thyroid dysfunction in GCA patients is linked to immune-mediated thyroid disease.

Main Methods:

  • A cross-sectional study comparing hypothyroidism and hyperthyroidism rates in 5,663 GCA patients versus 23,308 age- and gender-matched controls.
  • Statistical analyses included univariate (Chi-square, student t-test) and multivariate logistic regression models.

Main Results:

  • GCA patients exhibited significantly higher rates of hypothyroidism (18.2% vs. 6.91%) and hyperthyroidism (2.56% vs. 1.19%) compared to controls (p < 0.001 for both).
  • Multivariate analysis confirmed an independent association between GCA and hypothyroidism (OR 1.297, 95% CI 1.19-1.42).
  • No independent association was found between GCA and hyperthyroidism.

Conclusions:

  • Patients diagnosed with giant cell arteritis have a statistically significant higher proportion of hypothyroidism.
  • Clinical practice should include regular screening for thyroid dysfunction in patients with GCA.
