In prepubertal boys, a rise in testosterone levels correlates with a decrease in sex hormone-binding globulin (SHBG). This relationship was observed in boys with incomplete testicular descent and hypogonadotropic hypogonadism following hCG stimulation.

    Area of Science:

    • Pediatric Endocrinology
    • Reproductive Endocrinology
    • Hormone Physiology

    Background:

    • Sex hormone-binding globulin (SHBG) levels are influenced by androgens.
    • The response of SHBG to gonadotropin stimulation in prepubertal boys is not fully understood.
    • Testosterone production and SHBG regulation are key aspects of male reproductive development.

    Purpose of the Study:

    • To investigate the temporal relationship between steroidogenic responses and SHBG changes after human chorionic gonadotropin (hCG) administration in prepubertal boys.
    • To compare SHBG responses in boys with incomplete testicular descent versus those with hypogonadotropic hypogonadism (HH).

    Main Methods:

    • Studied 27 prepubertal boys (19 with incomplete testicular descent, 8 with HH).
    • Administered single or multiple intramuscular injections of hCG.

    Related Experiment Videos

  • Monitored serum testosterone and SHBG concentrations over time (daily for 5 days or up to 14 days post-treatment).
  • Main Results:

    • Boys with incomplete testicular descent showed a significant decrease in SHBG by day 14 post-hCG (P < 0.01).
    • Boys with HH exhibited impaired testosterone response to hCG, and their SHBG levels did not decrease.
    • SHBG decreased only in HH boys who had the highest testosterone responses.

    Conclusions:

    • Testosterone increase in prepubertal boys appears to be associated with a decrease in SHBG levels.
    • SHBG response to hCG stimulation is dependent on the degree of testosterone increase.
    • Findings suggest a regulatory link between testosterone production and SHBG levels during male puberty.
